Authorized services may include personal care, nursing care and skilled nursing visits.<\/p>\n<p>Algonquin Nurses Home Health Care I, LLC will direct and provide the services for your loved one. Algonquin Nurses is licensed with the State of MO, DHSS (Department of Health and Senior Services) to provide the special needs for your Child.<\/p>\n<p><strong>To be eligible for this program, the participant must:<\/strong><\/p>\n<ul>\n<li>Be a Missouri resident<\/li>\n<li>Be birth to age 21<\/li>\n<li>Need medically necessary services<\/li>\n<li>Be enrolled in MO HealthNet<\/li>\n<\/ul>\n<h3>What is a Service Coordination?<\/h3>\n<p>Special Health Care Needs, (SHCN) Public Health Nurse Service Coordinators monitor services through assessments, regular home visits, medical records and care plan. Algonquin Nurses Home Health Care and DHSS will coordinate these services.<\/p>\n<ul>\n<li>Evaluation and assessment of needs<\/li>\n<li>Identifying and accessing service providers<\/li>\n<li>Service plan development and implementation<\/li>\n<li>Transition planning<\/li>\n<li>Coordination of services through resource identification and referral<\/li>\n<li>Family Support<\/li>\n<li>Assisting in establishing a medical home<\/li>\n<\/ul>\n<p>Prior Authorization of medically necessary services is required, including<\/p>\n<ul>\n<li>Private Duty Nursing<\/li>\n<li>Advanced Personal
