A friend or family member may be hired as an attendant, with the exception of a spouse or anyone under the age of 18.<\/p>\n<p>With CDS, participants direct their own individual services and choices. This program is designed for consumers with disabilities and seniors in order that they may live independently in their own homes. Independent living skills training, advocacy, information and referral and peer counseling are integrated with the assistance of Algonquin Nurses Home Health Care I, LLC.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-151 alignright\" src=\"https:\/\/algonquinnursesstl.com\/wp-content\/uploads\/2020\/04\/service-1-4.jpg\" alt=\"\" width=\"368\" height=\"223\" srcset=\"https:\/\/algonquinnursesstl.com\/wp-content\/uploads\/2020\/04\/service-1-4.jpg 363w, https:\/\/algonquinnursesstl.com\/wp-content\/uploads\/2020\/04\/service-1-4-300x182.jpg 300w\" sizes=\"(max-width: 368px) 100vw, 368px\" \/><\/p>\n<p>To qualify for CDS services, a person must:<\/p>\n<ul>\n<li>Be receiving or eligible to receive Medicaid Benefits.<\/li>\n<li>At least 18 years of age<\/li>\n<li>Have a physical disability<\/li>\n<li>Be able and willing to recruit, hire and manage their own caregiver attendants<\/li>\n<li>Seeking to \/ or living in and independent living situation<\/li>\n<li>Be able to self-direct their care services<\/li>\n<\/ul>\n<p>The attendant must have a clean background check through the MO State Family Care Safety Registry before being employed by CDS a participant<\/p>\n<h3>What kind of help can I receive at home?<\/h3>\n<p>Through a State approved Plan of Care, an individual can have assistance with daily tasks such as:<\/p>\n<ul>\n<li>Meal Preparation<\/li>\n<li>Medication Reminders<\/li>\n<li>Personal Care<\/li>\n<li>Light Household Tasks<\/li>\n<li>Laundry<\/li>\n<li>Transportation for Shopping &amp; Errands<\/li>\n<\/ul>\n<h3>How It Works<\/h3>\n<p>The individual receiving Medicaid benefits will need to meet the Missouri Division of Senior and Disability Services (DSDS) requirements to qualify for caregiver services.<\/p>\n<p>DSDS will complete an assessment to determine the individual\u2019s needs and forward a personalized care plan to Algonquin Nurses Home Health Care I, LLC.
